U. S. Citizenship DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a Full-time , Permanent , Clinical Dietitian, Patient Aligned Care Team (PACT) . This position will be located in the Dietetics Service. The Clinical Dietitian is directly supervised by the Supervisor of Clinical Nutrition, under the Director of Patient Care Services/Nurse Executive. The Clinical Dietitian (PACT) may be scheduled and assigned to other nutrition practice areas, including Community Based Outpatient Clinics (CBOC's).

As a Clinical Dietitian, you are responsible for the following duties to! include, but not limited to:

The increasing complexity and sp! ecialization in nutrition requires that the dietitian have expertise in a wide range of disease entities, treatment modalities and drug/nutrient interactions.

The incumbent will be involved in the professional practice of dietetics in accordance with the guidance set forth in Nutrition and Food Service policies and procedures and VA regulations governing the practice of dietetics.

Conducts a comprehensive nutrition assessment for patients at nutritional risk.

Develops and implements an individualized plan for medical nutrition therapy in accordance with the patientÂ's

medical program goals and objectives. Accurately obtains and interprets medical information pertinent to the patientÂ's

nutritional care to determine risk, assess nutritional status and develop individual specialized care plans.

Calculates nutritional requirements (calories, protein, carbohydrates, lipids, vitamins, electrolytes, fluid) and necessary diet r! estrictions or food/drug interactions. Diets prescribed are tailored to each individual client according to his/her physical and metabolic needs. Nutritional treatment plans take into consideration the psychosocial, behavioral, ethnic and cultural factors to obtain desirable outcomes and promote long term adherence to a medical nutritional care plan.

Develops a therapeutic plan for specialized nutrition support, including enteral and parenteral nutrition for patients with complex medical and nutritional needs. Identifies patients in need of nutrition support and recommends type of support needed, suitable route, formula, and rate of administration. Provides initial and on-going nutritional assessments of patients receiving enteral and/or parenteral nutrition.

Utilizes assessment and evaluation techniques that consider age-specific needs as well as cultural, religious, and ethnic concerns. Orders labs, diets and consults per Scope of Practice/Clinical privi! leges.

Evaluates and monitors the effectiveness and outcome of! medical nutrition therapy interventions. Evaluates the patientÂ's response to care based on identified, measurable outcomes and reviews the plan as appropriate. Documents nutrition care plan in the patientÂ's medical record, including nutrition assessment data, medical nutrition therapy initiated and recommendations, results of nutrition counseling and discharge planning. Documentation supports an interdisciplinary approach and continuity of care. Works cooperatively with all medical center staff to provide comprehensive care to the patient.

Assesses educational needs and provides nutrition counseling for individuals and groups, incorporating a learning approach that considers the age, learning style, cultural, religious, educational, and medical situation of the patient.

Evaluates achievement of learning objectives by the patient and/or family. Adapts nutrition education materials and counseling techniques to clientÂ's level of understanding. Provides ap! propriate follow-up in accordance with patientÂ's treatment goals, referring patients for outpatient counseling, community or home health service as appropriate.

Assists in the education of dietetic students and interns. Plans learning experiences, teaches, and evaluates performance.

Participates in community projects, education, and VA committees as assigned.

Documents in the medical record the education provided, the care plan determined by the clinical dietitian, the medical provider and the client, an assessment of the clientÂ's comprehension of the education given and the expected compliance.

Provides nutrition care and education using video teleconferencing technology as assigned. Familiarity and experience with Telehealth technology including skills in trouble shooting in the use of Telehealth delivery methods of care.

Ability to communicate effectively via a variety of communications mediums with a broad range of clinical! , administrative, technical, community groups and individuals.

! Is an integral part of the interdisciplinary health care team, providing nutrition care to improve the health and well-being of the patient. Discusses pertinent findings and interprets nutritional information which will assist in developing comprehensive patient treatment plans. Participates in patient discharge planning.

Presents seminars, workshops, and lectures to convey current nutrition information on new and changing concepts as assigned. This includes the development and participation of in-service educational programs directed to members of interdisciplinary teams, Nutrition and Food Service personnel, nursing, students, interns, residents, etc.

The clinical dietitian will conduct scheduling activity for assigned clinics based on established scheduling policy and procedures. This includes, but is not limited to, application of scheduling priority guidelines, adherence to clinic profiles, consult management, designation of desired appointment date,! documentation of clinic cancellations and no-show activity.

The employee will update and/or utilize the Electronic Wait List (EWL) to accurately reflect patients waiting for scheduled appointments. The employee will comply with the mandatory training requirements for the outpatient scheduling process.

Maintains productivity standards and practices, effective time management, and prioritization of tasks. Enters data into the Event Capture System for work load capture and billing purposes within established time frames.

Completes Minimum Data Sets (MDS) as assigned and within established time frames.

Assists in developing and revising patient education materials, clinical forms, policies and procedures, and center circulars. Assists in achieving compliance with regulatory agency standards, including state, federal, and Joint Commission standards. Protects confidentiality of patient information.

Performs others duties as assigned t! o meet the needs of the department, including provision of support/cove! rage for vacancies and leave. Areas include but not limited to the MOVE Weight Management Program for Veterans, inpatient and outpatient, and community living centers.

Other duties as assigned.

Work Schedule: The tour of duty for this position is Monday-Friday, Administrative hours.

Relocation Expenses: Relocation expenses are not authorized for this position.

QUALIFICATIONS REQUIRED:
Basic Requirements:
Citizen of the United States.

Education: The individual must have earned a minimum of a bachelorÂ's degree from a U.S. regionally accredited college or university, and fulfilled each of the following criteria: (1) Completed a didactic program in dietetics accredited by the Commission on Accreditation for Dietetics Education (CADE) (A copy of your certification is required). (2) Completed a CADE accredited supervised practice program. (3) Passed a national examination administered by the ! Commission on Dietetic Registration (CDR), the credentialing branch of the American Dietetic Association (ADA).

Registration : (1) All applicants must be registered with the Commission on Dietetic Registration (CDR). (2) Non-registered applicants who otherwise meet the minimum requirements in the basic qualification standard may be appointed as a graduate dietitian under the authority of 38 U.S.C. § 7405 (a) (1) (D). (Failure to obtain CDR registration during that period is justification for termination of the temporary appointment. This may result in termination of employment).

Must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.

Grade Requirements:
GS-7 : Experience, Education, and Licensure - None beyond the basic requirements.

In addition, the candidate must demonstrate the following KSAs:

Ability to provide nutrition counseling to patient! s, family members and/or care givers considering psycho-social issues a! nd age appropriate guidelines, and to communicate effectively with other health care professionals.

Ability to plan and organize work consisting of multi-tasks and priorities under general supervision.

Basic knowledge of computer operations; software, hardware, terminology, security requirements, and capabilities.

Ability to synthesize nutritional assessment components to evaluate nutritional status and utilize principles of medical nutrition therapy using established practice standards and guidelines to deliver nutritional care.

Knowledge of basic food production and service which incorporates principles of sanitation, safety, and menu planning within a health care environment.

Knowledge of educational concepts, methods, training materials, and resources and the ability to implement existing education and training activities for Nutrition & Food Service employees. (NOTE: See exception to licensure requirement listed above).

GS-9: Experience or Education - At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 2 full years of progressively higher level graduate education, or masterÂ's or equivalent graduate degree from an accredited university or college in the field of dietetics or closely related field.

In addition, the candidate must demonstrate the following KSAs:

Ability to plan and organize work of considerable difficulty and responsibility under minimal supervision.

For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated:

Ability to participate in research and performance improvement studies.

Knowledge of, and ability to, apply the basic principles of enteral and parenteral nutrition support.

Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy in ! order to plan, develop, and independently coordinate medical nutrition ! therapy intervention, interdisciplinary care plans, and nutrition education activities.

GS-11: Experience or Education - At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 3 full years of progressively higher level graduate education, or a Ph.D. or equivalent doctoral degree from an accredited university or college in the field of dietetics or closely related field.

In addition, the candidate must demonstrate the following KSA:

Knowledge of basic investigative processes and techniques in order to participate in the design of studies, collect data, interpret findings, and translate results into written and oral communication.

For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated:

Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy for ! patients with complex medical conditions in order to plan, develop, and coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities.

Knowledge of complex areas of clinical nutrition including management of critically ill and malnourished patients, and ability to apply medical nutrition therapy to patients with multiple co-morbidities.

Ability to act as a mentor/consultant for health care providers including registered dietitians, dietetic interns, nurses, physicians, medical students, and allied health professionals.

Grade Determinations:
Note: The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-7 to GS-11 . The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the grade and step at which the candidate wi! ll be placed. The salary is based on the grade and step approved for th! e selected candidate.

PHYSICAL REQUIREMENTS: Physical lifting can be controlled by incumbent by segmenting batches in small quantities. It requires carrying paper and office supplies. Use of computer will be intermittent, multitasking with interruptions (area will include mostly standing and sitting with some short distant walking), and driving from the Community Based Outpatient Clinics (CBOC's) to the main facility in Sioux Falls and to the CBOC's.
