DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a Full-time , Permanent, Hospice and Palliative Care(HPC) Midlevel, (Physician Assistant or Nurse Practitioner) . This position will be located in the Extended Care and Rehab Service Line. As a Hospice and Palliative Care Midlevel, you are responsible for the following duties to include, but not limited to:
Functions autonomously within agreed upon scope of practice to manage patient care needs for those patients with severe debilitation of life threatening illnesses or who are at the end of life.
The Midlevel caseload is obtained through consultation from other providers.
The Midlevel leads the Palliative Care team and works closely with health care providers in the outpatient setting to include: Home Based Primary Care(HBPC), Community Based Outpatient ClinicÂ's(CBOC), primary care, specialty care clinics, community hospice programs and community health nurses; in the acute hospital setting, including ICU; and in the Community Living Center (CLC).
The Midlevel leads the HPC Care team to assure adherence to administrative and clinical functions of the program.
The Midlevel conducts peer reviews, implements service agreements, analyzes performance measures, serves on committees, and assists the HPC team in maintaining statistics and data required to meet HPC goals.
The Midlevel provides in-depth patient and family education to allow for informed decision-making, assists in identifying and managing patient/family conflicts related to goals of care plan.
The Midlevel conducts patient and family conferences to determine goals for patients.
Identifies symptoms causing physical and/or emotional distress and establishes plan of care.
The Midlevel utilizes advanced practice knowledge in Palliative Care to manage complex health care needs of patients with serious, advanced illnesses in the inpatient and outpatient settings.
The Midlevel actively participates in the daily management of hospice and palliative care Veterans in the CLC.
The Midlevel serves as consultant to attending medical teams in the acute care area for symptom management, establishing goals of care and communication on end of life issues.
NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. The Hospice and Palliative Care Midlevel, Physician Assistant or Nurse Practitioner reports to the Extended Care and Rehab Site Director and works in collaboration with his/her assigned team, however, assignments may be made to other areas for purposes of cross coverage.
Work Schedule: The tour of duty for this position is primarily Monday-Friday administrative hours. Evenings, Nights, Weekends and Holidays may be required.
Relocation Expenses: Relocation expenses are not authorized for this position.
QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS (Physician Assistant):
U.S. citizenship.
Proficient in written and spoken English.
Certification by the National Commission on Certification of Physician Assistants (NCCPA) is a requirement for employment. A Bachelor's Degree from a PA training program which is certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA);
OR
Graduation from a PA training program of at least 12 months duration (certified by ARC-PA) and a Bachelor's Degree in a health care occupation or health-related science;
OR
Graduation from a PA training program of at least 12 months duration (certified by ARC-PA) and a period of progressively responsible health care experience such as independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist or medical technician. The duration of approved academic training and health care experience must total at least 5 years.
Applicant's education and length of experience will be considered by a Professional Standards Board in determining the grade and salary of the applicant selected.

BASIC REQUIREMENTS (Nurse Practitioner):
Citizen of the United States.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English.
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration: collaboration, collegiality, Scientific Inquiry: quality of care, research.
Education/Experience Minimum Requirements:
 Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.  Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.  Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.  Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.  Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience. PHYSICAL REQUIREMENTS: The work is generally sedentary in nature with some daily intermittent walking, standing, bending, stooping, reaching, and carrying of lightweight items required. Rigid deadlines and need for high degree of accuracy periodically becomes stressful.
