VOLUNTEERS OF AMERICA, DAKOTAS POSITION TITLE: NEW START CASE MANAGER (exempt)PRIMARY PURPOSE/FUNCTION: Provide case management s! ervices and parenting education to pregnant or parenting adolescent females in a residential setting incorporating the use of Social Learning Theory as the primary philosophy.REPORTS TO: New Start Program CoordinatorPRIMARY DUTIES: Program DevelopmentA. Promote and coordinate development of support services between agency, schools, and community.B. Develop and maintain a working relationship with referral sources for purposes of providing information on diversity of services available.C. Assist the Program Coordinator and Group Life Supervisor in the development and review of Social Learning Theory program materials.Program ManagementA.
Ensure consistent and ongoing communication with immediate supervisor for program monitoring purposes.B. Complete timely and thorough documentation of client services and activities including intakes, progress reports, and monthly progress reports.C. Attend and participate in weekly staff meetings, providing a report on client progr! ess in parenting group and other pertinent information.D. Atte! nd trainings required to meet program licensing standards.E.
Ensure that client files are in compliance with all licensing standards.Client ServicesA. Assist in facilitating intakes and ensure all necessary paperwork is completed.B. Develop an individual treatment plan for each client.C. Solicit pertinent information regarding the educational, social, and emotional/behavioral histories of each client.D.
Ensure appropriate verbal and written communication regarding clients involvement in treatment to parents, referring workers, and Turning Point staff.E. Participate in staffings with consulting psychologist and psychiatrist as requested.F. Facilitate an educational group on a weekly basis as requested. Meet with clients individually to further address skills areas as needed.G.
Provide Aftercare services to clients as requested.H. Assist in ensuring the staff to client ratio by providing coverage when needed in the absence of Youth Supervisors, Grou! p Life Supervisor, and Program Coordinator.I. Participate in the clinical on call rotation.4. FacilityA.
Ensure high standards of cleanliness and maintenance of residential facility.B. Report facility needs to appropriate administrative staff.5. OtherA. Carry out other duties as requested due to program modification or expansion.HOURS: Part time position (30 hours / week) requiring flexible hours and rotation on an on call schedule.EXPERIENCE/EDUCATION: A Bachelors degree in social work or related human services field and a minimum of two years related work experience or an equivalent combination.
Must have experience and working knowledge of clients with mental health and/or chemical dependency issues. Experience working with pregnant and/or parenting teens is preferred.
