The VA Black Hills Health Care System (VA BHHCS) is located in the beautiful Black Hills of western South Dak! ota, where we are known for our blue skies, pine forests, rich history, moderate climate and friendly western hospitality. VA BHHCS provides primary and secondary medical and surgical care, along with residential rehabilitation treatment program (RRTP) services, extended nursing home care and tertiary psychiatric inpatient care services, for veterans residing in South Dakota and portions of Nebraska, North Dakota, Wyoming and Montana. Care is delivered through the Fort Meade (near Sturgis) and Hot Springs VA Medical Centers, as well as through a number of community-based outpatient and rural outreach clinics. The primary responsibility is coordination and provision of clinical pharmacy services to patients enrolled in a Mental Health program. The Clinical Pharmacy Specialist (CPS) will be a resource on mental health psychopharmacology for VA BHHCS as a whole. The Clinical Pharmacist Specialist, Pharmacy Department, applies clinical knowledge and skills when performing pharmaceutical functions and pat! ient care activities in the Pharmacy and Medical Center. Incumbent serves as a provider with prescribing privileges as outlined in the individualÂ's Scope of Practice for Clinical Pharmacy Specialist (CPS). The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both interaction with patients and providers. The pharmacist monitors and manages drug therapy for patients referred with chronic disease states (anticoagulation, etc.). The incumbent provides pharmaceutical care utilizing an advanced in-depth knowledge of current literature and contemporary practice of medications. This pharmacist promotes and manages drug therapy based on current clinical knowledge

that is consistent with policies established at the national, VISN, and local levels. The scope of this practice includes patient teaching, quality assurance, staff education and consultation, and is often in association with or in s! upport of pharmacists, providers, nurses, dietitians, and other health ! care practitioners. VA Black Hills supports a PGY1 and PGY2 Ambulatory Care Residency program.

Work Schedule: This position is part of the President's Mental Health Initiative program and may require working extended hours to include evenings and weekends.

BASIC REQUIREMENTS:

Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.

License, Registration, Certification Requirements (as applicable)

Completion of an ACPE-accredited Pharm.D. program

1 year of experience equivalent to the next lower grade (GS-11) level

Preferred experience: Mental Health and/or PGY1 and PGY2 residency.

GRADE DETERMINATIONS:
Pharma! cists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):

1. Comprehensive knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy such as quality assurance/utilization review, informatics systems, drug information, etc. This level of knowledge is usually characterized by at least 1 year of experience in a specialized clinical area or advanced training/certification.

2. Comprehensive knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, pharmacoeconomics and pharmacotherapeutics. This level of knowledge is usually characterized by at least 1 year of experience in a specialized clinical area or advanced training/certification.

3. Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters.

4. Knowledge of the design, conduct, and interpretation of contr! olled clinical drug trials or other research related to health care.
5.. Ability to communicate orally / in writing to persuade and influence clinical decisions

6. Comprehensive knowledge of DEA, FDA, VA, the State (Territory of District of Columbia, if appropriate), TJC and other standards related to the distribution and control of scheduled and non - scheduled drugs to both inpatients and outpatients (including research and investigational drugs).

7. Ability to manage people and/or programs.
