U. S. Citizenship DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a Full-time , Permanent, Supervisory Pharmacist (Inpatient) . This position will be located in the Pharmacy Service. As a Supervisory! Pharmacist (Inpatient), you are responsible for the following duties t! o include, but not limited to:

Supervisory Functions:
Supervises all inpatient staff including clinical pharmacists, clinical pharmacy specialists and inpatient technicians.
Effectively organizes and assigns workload, solves problems, and achieves established goals.
Leads the interview process and screens applicants for inpatient pharmacy positions. Evaluates qualifications, interview ratings, and makes recommendations to the Chief, Pharmacy regarding employability of such candidates.
Implements orientation and training programs for inpatient staff.
Develops policies and procedures relative to the assigned area.
Establishes work schedules and allocates staff as needed to provide adequate coverage. Maintains time and attendance records and approves leave for direct reports.
Evaluates the performance of direct reports and seeks opportunities to help these individuals continue to improve their overall performance.
Assists with th! e development and maintenance of functional statements, performance standards, and competencies for direct reports. Establishes appropriate performance measures for inpatient pharmacy services, monitors and tracks performance, and provides reports as assigned.
Conducts performance appraisals for inpatient staff.
Works collaboratively with other supervisory staff to assure that all patient care needs are met through staffing plans and daily interactions.
Assures that all regulatory requirements for hospital standards (JC, SOARS, OIG, AWE, etc.) are current and in compliance.

Clinical Functions:
The inpatient pharmacist provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or ! recommendations to other health care providers in charge of the patient! both orally and in writing. Monitoring will include prospective review and intervention in:

  • Therapeutic appropriateness of a patientÂ's drug regimen.
  • Therapeutic duplication in the patientÂ's drug regimen.
  • Appropriateness of the route and method of administration.
  • Degree of patient compliance with the prescribed drug regimen.
  • Drug-drug, drug-food, drug-laboratory, or drug disease interactions.
  • Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects.
  • Physical signs and clinical symptoms relevant to the patientÂ's drug therapy.
Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
Provides medication counseling to patients including, but not limited to, any changes made in the current medication regimen.
Obtains medication histories from patients, when appropriate, a! nd summarizes the significant findings. These findings are documented in the medical record and reported to the primary physician.
Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients and caregivers.
Participates in patient care by reviewing charts, evaluating pertinent laboratory data, drug-drug and drug-nutrient interactions, monitoring for adverse drug effects, screening for allergies and may participate in patient care rounds.
Performs medication reconciliation at transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking.
Assesses drug safety and efficacy, including evaluation of physical symptoms.
Reports adverse drug events (ADE), near misses, and medication errors in alignment wit! h VA ADERS reporting program.
Participates in the medical center me! dication utilization evaluation (MUE) program.
Recommends formulary alternatives and completes non-formulary consults as assigned.
Participates in emergency preparedness activities.
Provides pharmacokinetic consultation, including dosing vancomycin and aminoglycosides, in accordance with local policy.
Implements P&T Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education.
Manages recalls and medication shortage situations by substituting alternate dosage strengths and instructions of the same medication to equal the prescribed dose and schedule.
Enters supply orders appropriate for patient care per facility policy.
Manages various actions, as appropriate, for medication orders to include: flagging prescriptions, managing view alerts, using the Â"hold medicationÂ" function, and generating unsigned discontinued orders using Service Reject.
Extends refills until next! scheduled appointment under the providerÂ's name per facility policy.
Serves as a role model and preceptor to pharmacy students and residents.
Assumes an active role in staff development of peers and provides educational in-services to nursing, medical staff or to other ancillary services.
The pharmacist is responsible for maintaining his/her professional competency by keeping abreast of current medical and pharmaceutical literature, new drugs and therapies, and applies this knowledge in his/her daily duties.

Educational Functions:
Provides education to health care providers regarding medication use.
Maintains knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice.
Serves as the Inpatient Pharmacy Residency Preceptor.
Assumes a leadership role in the overall inpatient pharmacy-training program consisting of students, residents, and professional and non-professional staff m! embers. Conducts on-the-job training and provides guidance through desi! gnated programs.
Keeps abreast of new developments in pharmacy practice and drug therapy by reading pharmacy and medical literature, and participating in continuing education programs.

Work Schedule: The tour of duty for this position is typically Monday - Friday, administrative hours, with occasional nights, weekends, and holidays (as needed for purposes of coverage).
Relocation Expenses: Relocation expenses are not authorized for this position. QUALIFICATIONS REQUIRED:

Basic Requirements:
Citizen of the United States.

Education
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
Licensure
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico)! , or the District of Columbia.
Must be proficient in spoken and written English.

Grade Requirements:GS-13:
Experience
  • Applicants must have one (1) year of specialized experience equivalent to the GS-12 level. Specialized experience is experience that equipped the applicant with particular knowledge, skills, and abilities (KSA's) to perform successfully the duties of the position.
Pharmacy Residency Preceptor Requirements:
In addition to the basic requirements listed above, applicants must also qualify to be a Pharmacy Residency Preceptor . To qualify to be a Pharmacy Residency Preceptor, you must meet the following 2 requirements. Residency Requirement
Preceptors must be licensed pharmacists who have completed an ASHP-accr! edited residency followed by a minimum of one year of pharmacy practice! experience OR;
Alternatively, licensed pharmacists who have not completed an ASHP-accredited residency may be preceptors but must demonstrate mastery of the knowledge, skills, attitudes, and abilities expected of one who has completed a PGY1 residency and have a minimum of three years of pharmacy practice experience.
2. Contribution and Commitment to Pharmacy Practice
Preceptors must have a record of contribution and commitment to pharmacy practice characterized by a minimum of four (4) of the following seven (7) accomplishments:

a. Documented record of improvements in and contributions to the respective area of advanced pharmacy practice (e.g., implementation of a new service, active participation on a committee/task force resulting in practice improvement, development of treatment guidelines/protocols).
b. Appointments to appropriate drug policy and other committees of the department/organization.
c. Formal recognition by peers as a model pra! ctitioner (e.g., board certification, fellow status).
d. A sustained record of contributing to the total body of knowledge in pharmacy practice through publications in professional journals and/or presentations at professional meetings.
e. Serving regularly as a reviewer of contributed papers or manuscripts submitted for publication.
f. Demonstrated leadership in advancing the profession of pharmacy through active participation in professional organizations at the local, state, and national levels.
g. Demonstrated effectiveness in teaching (e.g., through student and/or resident evaluations, teaching awards).

Grade Determination:
Note: The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the step at which the candidate will be placed in that specific grade. The salary is based on the grade and step approved for the selected candidate.
