The Sioux Falls VA Health Care System is currently recruiting for a Full-time , Permanent, Registered Nurse-Community Health Nurse Coordinator . This p! osition will be located in Home and Community Based Care Program in the! Extended Care and Rehabilitation Service Line. As a Registered Nurse you will be responsible for the following major duties to include, but not limited to:
The Community Health Nurse Coordinator (CHNC) plans, organizes and coordinates discharge needs for Veterans who require alternative community programs which include processing all consults requesting in home or community care services.
Functions include collaboration with all levels of nursing personnel, providers, social workers and other services involved in an individual VeteranÂ's care to establish continuity of care for Veterans after discharge.
The incumbent collaborates and coordinates with community resources for the purpose of providing care, as needed, in the community. This includes follow up on the quality of care that is being provided via satisfaction surveys and monitoring of Quality Indicators.
Obtain knowledge regarding VA, federal, state and local agencies and entitlement/resources and ! how to access and coordinate those services.
The incumbent needs to have knowledge of disease processes, disabilities, medications, terminal illnesses, medical economics and reimbursement processes.
Must be able to assess and interpret data about the VeteranÂ's social, emotional, mental, and physical health and to provide the care needed.
The incumbent will be involved in local and Network committees.
Needs strong critical thinking and problem solving skills.
Excellent telephone, interpersonal and communication skills required.
Possess ability to establish and maintain liaison through collaboration with local, state and federal agencies.
Highly motivated and committed to quality care.
Other CHNC functions may include Contract Nursing Home oversight visits/inspections, which may include Adult Day Health Care at scheduled intervals.
NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. ! The Registered Nurse reports to the Home and Community Based Care Progr! am Manager and works in collaboration with his/her assigned CORE team, however, assignments may be made to other areas for purposes of cross coverage.
Work Schedule: The tour of duty for this position is primarily Monday-Friday, Days, Administrative Hours.
Travel will be required and will occur during duty hours using a Government owned vehicle. This would be to conduct initial and periodic inspections of Contract Nursing Home and Contract Adult Day Health Care facilities as the nurse member of the VA Community Care Team and associated tasks in service to the Veteran, as needed, at both scheduled and unscheduled times during the tour of duty.
Relocation Expenses: Relocation expenses are not authorized for this position. QUALIFICATIONS REQUIRED:
Citizen of the United States.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at ! the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
- The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Must be proficient in spoken and writte! n English.
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

Education/Experience Minimum Requirement! s:
 Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
 Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
 Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
 Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doct! oral degree in nursing or meets basic requirements for appointment and ! has doctoral degree in a related field with no experience.
 Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

PHYSICAL REQUIREMENTS: Current Valid and unrestricted driver's license is required for this position. The work is generally sedentary in nature with daily intermittent walking, standing, bending, stooping, reaching, and carrying of lightweight items required. The incumbent will need to possess and maintain a current valid and unrestricted driver's license. Any driving that will be required with duties will encompass a ! large geographic area when conducting site visits to Contract Nursing Homes and Contract Adult Day Health Care facilities. Periods of time may be moderately demanding to include increased driving, between various locations. Intermittent complicated issues with Veteran or Caregiver may periodically become stressful. Verbal communication used frequently in contact with Veterans/ family/significant others/other key stakeholders via telephone. Documentation in electronic medical record is required for position necessitating good hand/eye coordination. Rare lifting or moving of items weighing < 20 lbs. may occur during duty hours. Work is typically performed in a clinical setting that exposes the incumbent to the communicable diseases common of the client population. In this role, work can be performed outside the medical center, doing site visits/periodic inspections of facilities where the environment cannot be controlled. The CHNC practices, using professional judgment and! expertise to make decisions about services and treatment provided to V! eterans.
