The clinical duties include, but are not limited to, carrying out the directives of the agency`s psychiatrist(s) for management of medications for consumers; provide consumer education on psychotropic medications; medication monitoring and administration; maintain security, inventory and accountability of all sample medications furnished to, and prescribed by, the psychiatrist(s); educate and monitor CCS team staff regarding medical services they are allowed to provide under nurse supervision: Prov! ide symptom assessment and management; crisis management and i! ntervention; liaision services; and case management. Assuring medical supports for the effective and efficient use of psychiatric services provided by professionals in the community is expected to facilitate the integration of behavioral and physical health services to the fullest extent possible. Participating in the on-call rotation of the agency is required.
The job of staff Nurse Health Home Coordinator is a clinical, direct service position. the job consists of assessing and developing individualized care plan developed by the designated provider with active participation of all health care team members. Employer expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging consumers at their current state of recovery. Services provided will be in accordance with the plans and guidelines as established by employer and the State Division of Mental Health and the State Board of Nursing.
Possession of an associ! ates or bachelor`s degree in nursing is required. LPN or RN required. Valid SD driver`s license, LPN or Registered Nurse license in South Dakota. Knowledge of: principles and practices of effective health care coordination, development, and evaluation; psychiatric nursing treatment and psychotherapeutic interventions; theories, techniques, and practices of professional nursing; principles, practices, and laws of professional nursing; basic occupational therapy and social work; laws and regulations pertaining to psychiatric nursing; the uses and effects of psychotropic drugs and narcotics; patient safety practices; theories, principles, and techniques of group and individual psychotherapy; symptoms and behavior patterns of emotionally disturbed and mentally ill patients; mental health community resources and services; health education and interviewing and evaluation techniques; common mental and physical illnesses, diseases, disabilities, injuries, and nutrition; medical equ! ipment operation.Ability to: Observe emotional and physical behavior of! patients; evaluate and assess the need for health services; analyze a situation and adopt a course of action; prepare and maintain medical records and reports; recognize and respect limits of confidential information; communicate with people of diverse socioeconomic and ethnic origins; speak before groups; exercise independent judgment in medical emergencies; demonstrate tact, diplomacy, patience and compassion; establish and maintain working relationships with other health professionals, subordinate staff, co-workers, other agencies, and organizations; comply with and enforce nursing policies, protocols, and program regulations; understand and follow instructions, work independently.
