U. S. Citizenship DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a Full Time, Permanent, Licensed Practical Nurse (LPN). This position will be located at the Watertown Community Based Outpatient Clinic (CBOC) in the Primary and Specialty Medicine Service Line. The Licensed Practical Nurse works under the direction of an RN and reports to the Primary Care, Nurse Manager. The LPN is responsible for observing, recording and reporting changes in the patientÂ's condition, as well as assisting in the development and revision of the patient-driven holistic care plan f! or life. The LPN is a member of an expanded team. The LPN requires a ge! neral review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician) and Expanded Team (family/care giver, internal and community based services involved in providing care to the patient). As an LPN, you are responsible for the following main duties to include, but are not limited to:
Demonstrate knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
Recognize the need for and institute emergency measures when indicated, promptly seek assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and /or pulmonary arrest.
Ability to observe, identify and respond to the patientÂ's needs for nursing interventions and recognizes the need to consider emotion! al, spiritual, socio-economic and age related factors.
In collaboration with the RN, participate in the assessment, planning, implementation, and evaluation of the nursing care that expedites the patient's return to, or maintenance of, optimal functioning.
Possess the ability to evaluate patientÂ's status and interpret the appropriate information to identify each patientÂ's age-specific needs and to provide the identified care.
Report and record observations and information related to the care of the patient and integrate the use of health care technology into the provision of nursing care.
Establishes constructive relationships with individual patient and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills.
Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspect of care delivery.
Effectively! incorporates knowledge and understanding of established customer servi! ce standards in all interactions with patients, family members, and/or other customers.
Actively seeks out education opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
Work Schedule: The tour of duty for this position is Monday - Friday, Days.
Relocation Expenses: Relocation expenses are NOT authorized for this position. QUALIFICATIONS REQUIRED:
Basic Requirements:
Citizen of the United States.
Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
Applicant must possess and maintain a full, active, current, and unrestricted license as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United S! tates or in the District of Columbia.
Must be proficient in spoken and written English.
Grade Requirements:
GS-3:
- None beyond the Basic Requirements.
Education/Experience Required:
- Six months of qualifying experience as an LPN or LVN; OR
- Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; OR
- Graduation from an approved school of at least 24 months duration.
Experience Required:
- Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.
Experience Required:
- Completion of at least 1 year of qu! alifying experience at the GS-5 level or equivalent.
Note: The full performance level of this vacancy is 06. The actual grade at which an applicant may be selected for this vacancy is in the range of 03 to 06. The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.

PHYSICAL REQUIREMENTS: The work requires some standing, walking, bending, lifting, and carrying of light items. The work requires agility and dexterity and may require heavy lifting of equipment or patients. In many situat! ions, the duration of the activity (such as most of a workday) contributes to the arduous nature of the job. Duties require extensive periods of standing and walking. Working with patients requires regular and recurring bending, lifting, stooping, stretching, and similar activities.
