U. S. Citizenship DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a Full-time, Permanent, Clinical Pharmacy Specialist (PACT). This position will be located in the Pharmacy Service. As a PACT Clinical Pharmacy Specialist, you are responsible for the following duties to include, but not limited to:
Assures the safe and appropriate use of medications, and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; evaluation, dispensing and providing medication! s; and drug information.
Communication and interpersonal relat! ions, including the ability to appropriately and courteously relate to internal and external customers. Maintains confidentiality of patient/employee information, electronic and print. Must work and function in the medical center in a safe manner, including infection control and the correct operation of equipment.
Must be able to communicate effectively with patients who are 66 years and older and who may have hearing, physical or cognitive impairments.
The Clinical Pharmacy Specialist is directly involved in the design, implementation and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. Provides patient -specific therapeutic drug monitoring and communicates relevant findings and appropriate treatment plans to the patient aligned care team. Monitoring will include prospective review and intervention in; therapeutic appropriateness of a patientÂ's drug regimen, appro! priateness of the route and method of administration, degree of patient adherence with the prescribed drug regimen, and drug-drug, drug-food, drug-laboratory, or drug disease interactions.
Ordering and monitoring laboratory data to evaluate the efficacy of drug therapy and to anticipate toxicity or adverse effects.
Performs continuous evaluation of prescribed medications to assure optimal drug therapy. Documents clinical interventions in a timely and professional manner. Provides medication counseling to patients including, but not limited to, any changes made in the current medication regimen. Able to adjust communication and tracking methods based on age and developmental considerations.
Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required and communicates findings with prescribers and pr! ovides appropriate alternatives or designs appropriate treatment plans ! as needed. Has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
Promotes and monitors compliance with established drug therapy policies. Works with providers to ensure compliance with national, VISN, and local initiatives.
Functions as a non-physician provider (mid-level) with an independent scope of practice: Initiates, modify or discontinue patientÂ's medication therapy for intensive medication therapy management services. Provides patient-specific drug therapy modifications to maximize patient response and minimize drug interactions, adverse drug effects, and polypharmacy while providing cost-effective treatment. The assessments are based on clinical reviews and objective measures.
Orders, performs, reviews and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and suppor! t the patientÂ's drug therapy. Provides follow-up with patients on lab or test results to discuss the plan of therapy (i.e. changes in medication therapy, monitoring and evaluation, additional testing requirements, referral to primary care or specialty care physician. Primary provider for extending medication therapy until next medical appointment, when patients are out of medications (limited to non-controlled substances) in case-by-case situations. Evaluates and orders supply items as appropriate for patient care. Serves as an authority on drug usage, interactions, over-dosages, and adherence to medical staff, nursing staff, technicians, and students. Uses appropriate references to research drug information.
Performs medication reconciliation at transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. his may include adding non-VA medications or discontinuing duplicate medications or those th! e patient is not taking. Reports adverse drug events (ADE), near misses! , and medication errors in alignment with VA ADERS reporting program. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient adherence issues.
Participates in the medical center medication utilization evaluation (MUE) program.
Recommends formulary alternatives and completes non-formulary consults as assigned.
Participates in emergency preparedness activities. Provides pharmacokinetic consultations. Implements P&T Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education.
Recommends therapeutic alternatives to providers during medication recalls or shortages.
Serves as a role model and preceptor to pharmacy students and residents. Assumes an active role in staff development of peers and provides educational opportunities to nursing, medical staff or to other ancillary services. The clinical pharmacy specialist is responsible! for maintaining his/her professional competency by keeping abreast of current medical and pharmaceutical literature, new drugs and therapies, and applies this knowledge in his/her daily duties.
Actively participates in committees and projects as assigned.
Other duties as assigned.
Work Schedule: Rotating shifts will include; days, nights, weekends and holidays; flexibility in tours and days of the week will be required.
Relocation Expenses: Relocation expenses are not authorized for this position. QUALIFICATIONS REQUIRED:
Basic Requirements:
Citizen of the United States.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Ed! ucation through their Web site at: http://www.acpe-accredit.org/
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Must be proficient in spoken and written English.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or sub standard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professiona! l incompetence, or sub standard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
Grade Requirements:
GS-13: Experience - In addition to the basic requirements, candidates must meet the following:
One year of experience equivalent to the next lower grade level, (GS-12).
The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined ro! le in budgetary execution and serves as a mid-level provider who functi! ons to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
Demonstrated Knowledge, Skills, and Abilities (KSAs):
Applicants eligible for the GS-13 level must meet all basic requirements above. The KSAs shown below apply to this assignment for this grade level:
a. Ability to communicate orall! y and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Grade Determinations:
Note: The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.
Completion of an ASHP Accredited Residency is preferred. !
